About the Role

An exciting opportunity has arisen for a Social Care Support Officer to join our newly created Intermediate Care Allocation Team.


You will be an integral part of a team working in partnership with health and social care colleagues across the Morecambe Bay area, serving the citizens of Lancashire.

The initial phase of this exciting new service will be a focus upon the 'home first' discharge to assess pathway, with a vision of the service then further developing to include management of referrals to access residential intermediate care. Through collaborative working with occupational therapy, physiotherapy, nurses, social care workers and through holistic screening, appropriate and time limited intermediate care services will be commissioned to ensure service users’ needs are met. 


The role will involve conducting face to face strengths based assessments with people in their own homes, helping people to receive the level of support that is right for them at that time, and enabling them to achieve their optimum level of independence and live their life in a way that they choose. You will be bringing the ethos and principles of Passport to Independence to the team, and supporting the team to work in an outcome focussed way.


The team are based in Lancaster and work across 7 days, so some weekend working will be required. You will receive regular support from the team along with professional supervision. There will be opportunities to build on existing skills and knowledge to enable you to flourish as part of an exciting new team that is looking to bring real benefits to the Morecambe Bay Area.
