An exciting opportunity has arisen for a Social Worker to join our Community Mental Health Team.

As part of this team you will complete social care assessments, support plans and reviews designed to support the delivery of a coordinated response to individual's needs.  The team works in partnership with all internal and external professionals involved in assessment and service provision, eg. Health, police, housing etc. including Court work when required.

You will need to be someone who can provide professional supervision and oversight to members of the team as well as contribute to the provision of high quality assessments, reviews and support plans for individuals.

There is a full time role working across Monday to Friday.

Candidates must hold a relevant qualification in social work and be registered with SW England.

You must be registered (SWE) to be appointed as a Social Worker or have applied to register with the Social Care Council. For Social Worker/Care Manager positions where non social worker qualifications are accepted, registration is not required, and you will be appointed as a Care Manager.
