Social Worker – Mental Health Integrated Discharge Team

Are you ready for a fresh challenge? Do you wish to find a supportive and team focused environment to flourish in? Are you looking to help maximise outcomes for people in your community?

Lancashire Adult Social Care has transformed how we assess, deliver and support our citizens in Lancashire with a significant focus on promoting and maintaining independence through creative support planning.

We have some exciting opportunities shown below for two social workers to come and join Adult Social Care in our Mental Health Integrated Discharge Team.

The posts on offer are based across Lancashire:

  •  Contact Debbie Honeysett on 07917836698 for an informal discussion of the role.

We're looking for people who are able to display the following skills and experience:

  • Someone who is able to undertake high quality assessments, reviews and support plans for individuals.
  • Someone who can contribute to effective team working and to service development and to work with management to identify improvements that could be introduced to enhance the efficiency and effectiveness of the service.
  • Someone who can build and sustain relationships to work collaboratively with colleagues and external partners involved in service provision, e.g. Health, Police, Housing and Court as required, whilst having confidence to challenge what is not working and work creatively to suggest alternative proposals.
  • Someone who can work under pressure, manage their time flexibly, intelligently under multiple demands and prioritise and manage a caseload.

We have a lot to offer people who join our Adult Social Care Teams including a comprehensive two-week induction programme in our bespoke Adult Social Care Academy which also includes four additional recall training days throughout your first year with us. We follow this up with a supportive framework, regular supervision with managers and opportunities to build on your existing skills and knowledge.

If you think you can bring something to this Service and the Team that makes it work, please use the documents below to support you in writing your application:
