An exciting opportunity has become available for a Social Worker to join our Hospital Discharge Team in Central Lancashire.

 

About our team

We are an established team within the Acute service within Adult Social Care at Lancashire County Council comprising of Senior Social Workers and Social Workers.

We work together to  support  the decision making required to progress discharges onto our Discharge to assess pathways to facilitate safe and timely discharges whilst maintaining a commitment to promoting independence in supporting our citizens in Central Lancashire to achieve positive outcomes. This work is currently completed virtually although some office presence is required.

 

The role

In this Social Work role you will complete Care Act assessments and participate in Multi Disciplinary meetings to consider eligibility for Continuing Health Care for individuals who have been discharged from hospital with Discharge Support funding.  These individuals will typically be in a bed based setting .

You will work to defined timescales and promote interventions based upon the client's strengths and focus on what is important to them, offering a truly holistic strength based approach. You will have the ability to engage with service users, their carers and system partners to undertake these assessments virtually and to commission appropriate care to promote independence and meet identified needs.

The post on offer is a 12 month temporary contract based on a 7 day working pattern so there will be a need for you to work some weekends.

We are looking for an enthusiastic Social Workers with a passion for promoting independence and supporting the Discharge to Assess model.

You will need to be flexible and adaptable in a constantly changing environment as well as having a commitment to improve the lives of our service users through creative, evidence based practice.

You must be registered (HCPC) to be appointed as a Social Worker or have applied to register with the Social Care Council. For Social Worker/Care Manager positions where non social worker qualifications are accepted, registration is not required and you will be appointed as a Care Manager.

The post you are applying for is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (as amended in 2013).
