Social Worker

The Intermediate Care Allocation Team in East Lancashire is a well-established integrated multi-disciplinary team based at St Peter’s Centre in Burnley. Over the years our team, functions and operating hours have expanded. Primarily, we are a single point of access for a range of community based intermediate health and social care services. 

 

We are looking to recruit to a full time (37 hours per week), permanent Social Worker. This post is a 7 day posts and there is a requirement to work 1 weekend in 4, enhancement for which are paid and time off in lieu is given through the week.

 

We welcome applications from Social Workers with experience, those who are newly qualified or those who are due to qualify/awaiting registration. Please note if you have completed your course but are awaiting formal award/registration we can commence employment on grade 6 pay scale until the Social Worker England registration comes through.

 

The role will predominantly be working within the East Lancashire area of Burnley, Pendle Hyndburn, Ribble Valley and Rossendale, with a base within St Peter’s Centre in Burnley, but you will be provided with the equipment to work from home.

 

Our main functions are as follows:

Provide a single point of access so that referring groups can make one referral for intermediate care services rather than multiple referrals to several services.

Triage referrals and make decisions and recommendations based on a trusted assessment to ensure  people who need our services get them in a timely way.

Organise rapid access to services to avoid unnecessary admissions to hospital or care home.

Support the holistic wellbeing of the people who use our service to get them back on their feet following a period of illness.

Provide holistic assessment, review , case management and care co-ordination functions following intervention to step people down (or up) to the right service according to their needs.

 

The Social Work position within ICAT will be required to:

  • Contribute to front line duties of the team.
  • Carry a small but rapidly changing case load.
  • Support Student Learners by offering placements and support other students within the team.
  • Understand and apply the Care Act.
  • Understand and apply the Mental Capacity Act.
  • Understand and apply the Human Rights Act.
  • Ensure and embed the principals of Passport to Independence are maintained within your day to day practice and decision making.
  • Support the wider team by offering specialist advice and support on all social care matters.
  • Utilise and share your social work knowledge and skills in relation to theories, models and methods to improve the overall practice of team members.
  • Promote and embed social work values with non-social work colleagues.
  • Deal with low level/informal complaints and feedback as well as support the manager with more complex complaints to achieve an amicable outcome.
  • Take a lead on proactively promoting the reablement service and work closely with the reablement team to improve practice and identify and implement opportunities to improve the service we provide.
  • Provide a social work role in ensuring good multi-disciplinary working occurs within the Community Beds to ensure the best possible outcome for the people who use that service.
  • Contribute to service developments that would improve the overall performance of the team. 

 

The ideal candidate will have the following prerequisites:

  • A working knowledge of the Intermediate Care system and how that fits with both pre and post hospital care.
  • Experience of working in Intermediate Care settings with a range of people with varying needs.
  • Be willing and able to complete the ASYE if applicable.
  • Be willing to work towards Practice Educator status if applicable.
